When the Phantom Event is active, the entire map transforms into phantasmal ruins. Here's what changes:
- Two upside-down phantasmal, translucent black buildings on floating islands flank the sides of the map
- A singular phantom pillar and two smaller phantom floating islands near each building
- Phantom pillars adorn the map corners
- A giant phantasmal floating pillar behind the Cave's entrance and exit
- The Cave's gateway is adorned with a black arc
- Windows on buildings, engravings on pillars, and the arc glow white periodically – in sync with the eyes of Phantom Mutation Brainrots

The Phantom Mutation is a rare mutation that can only appear on Brainrots spawned during the Phantom Event. Its unique features include:
- Blackout design – the Brainrot appears completely black/translucent
- Glowing white eyes that pulse periodically
